PERMISSION SLIP
For the performance of the Sleeping Beauty at the Palace Theatre on June 9, 2012 at 2:00pm or 7:00pm
I, ____________________________ (parent/guardian) give my permission for _______________________(dancer)to participate in performance, and acknowledge and agree to abide by the following terms and conditions:
Section I – Laurel Youth Ballet Theatre (school)
1) Choice of performance times is the artistic director’s – as is number of performances, type of costume and type of shoe is worn to perform.
2) Dancer must attend the proper number of classes per week per level
(Company level class is split into 2 levels – 5a requires 4 classes/wk – 5b requires 5 classes/wk).
A. You may not miss excessive amount of either class or rehearsals.
B. You will be notified in writing if you are missing too much class or rehearsal. Your written notification will state your options.
C. Your account must be paid up-to-date in order to attend class, rehearsals and therefore perform.
3) You may not miss any on stage rehearsal and must attend your scheduled on stage rehearsal.
4) Extenuating circumstances may be accepted by the artistic director but there is no guarantee that the requirements will be suspended or altered.
5) You must be available for “all call” rehearsals the week prior to performance.
6) You are responsible for any damage done to the costume while in your possession and must pay replacement or repair costs if the costume is damaged.
7) You must be aware of and follow the performance rules.
8) Deviation from the rules will result in removal from performance.
Section II – Laurel Ballet Performing Company
1) To pay the performance fee (by level) by February 28th to LBPC. Failure to do so will result in suspension from rehearsals and costuming, which will jeopardize participation in performance.
2) The performance fee is refundable at 75% until April 30, 2012. No refunds after that date. A request for a refund is considered to be a withdrawal from all rehearsals and performances.
3) To abide by the rules and specifications of the performance site at all rehearsals and performances.
4) To pay for program advertisements in full upon ordering, or they will not appear in the program. All signs, posters, advertisements or photos to be posted in the theater during rehearsals and performances
will be created by LBPC and/or its sponsors in cooperation with LPBC.
5) That all performance fees, program advertisements, and mandatory fund raising are the responsibility of LBPC, and that any other issues will be directed to Laurel Ballet School.
